Intake Forms

Fill out or update your intake forms

Want to determine if your child is a candidate for intensive therapy? The first step is to fill out our intake paperwork online. Make sure you have your most current OT, PT, and speech evaluation on hand, so you can upload them before submitting this form. If you have insurance you would like to be reviewed, please include an uploaded picture of the front and back of your insurance card.

After you submit your paperwork, please allow up to two weeks for our therapists to review your intake and determine if your child is a candidate for our intensive program. At this time, they will decide what type of therapy and how many hours should be included in your individualized plan. Our intensive coordinator will send you an email with our finalized therapy recommendations.

Session Registration

After we know what type of therapy and how many hours are appropriate for your child, we can move forward with scheduling. Click below to fill out the Intensive Request Form for the session of your choice. At this time, you can also request an intensive cost breakdown, which is an estimate of what we anticipate insurance will pay regarding your specific recommended therapy plan.

Place a deposit

After we receive your Intensive Request form, our intensive coordinator will reach out to you with a possible opening for the session you requested. Please note, filling out this form lets us know that you are interested in a specific session. It does not guarantee you a spot on our schedule. Your spot is confirmed once you place a $1,000 deposit for the tentative schedule provided by the intensive coordinator. If the session you requested is all booked up, we will add you to the waitlist for that session date and send you an email confirmation.

We are currently booking for 2018. We offer our sessions on a first-come, first-served basis. The booking request form will become active via the link below once registration begins.

Why register in advance?

Reserve your child’s space in the session that works best with your schedule. As you can imagine, our summer sessions are by far our most sought after. The only way to secure your space is to register while they are still available!

Does advanced registration guarantee your therapy team preferences?

The earlier you register, the better we will be able to work around your preferences. However, your therapy team is not confirmed until 30 days prior to the session start date when the final schedule is released. For consistency purposes, we do our best to schedule your child with therapists they have worked with in the past, but we cannot guarantee that each of your therapists will be available during the full three-weeks of any particular session. Similar to you, our therapists have family emergencies, become sick, or need a vacation from time to time. We will do our absolute best to inform you of any schedule changes well in advance. If one of your therapists is out during a portion of your session, we will provide you with your alternate schedule 3-4 weeks prior to the start of the session.

What is your cancellation policy?

Outside 30 days from session start date: If you would like to move your deposit to a new session you can do so for a $250 fee. This fee may be waived if you need to switch for a medical purpose (injury, illness, or anything that prevents your child from participating in therapy) but you must provide documentation from your child’s physician. Within 30 days from session start date: If you would like to move your full payment to a new session we can do so for a $1000 fee. This fee may be waived if you need to switch for a medical purpose (injury, illness, or anything that prevents your child from participating in therapy) but you must provide documentation from your child’s physician.
